2. Intake Information and Referral Consent

By submitting information through this intake form, you consent to NAVF collecting, processing, and securely storing the personal, medical, financial, benefit-related, and other information you choose to provide.

NAVF uses this information to better understand your situation, provide general education and navigation support, identify possible next steps, and help connect you with appropriate resources or referral options.

NAVF does not provide legal, financial, tax, medical, or VA-accredited claims representation. NAVF does not guarantee eligibility, approval, benefit amounts, or processing times.

If claim preparation, claim submission, or representation before the Department of Veterans Affairs is needed, those services must be provided by an appropriate VA-accredited representative, attorney, claims agent, or Veterans Service Organization.

If NAVF determines that a referral may be appropriate, and you consent to that referral, NAVF may share your intake information and documents with the third-party professional, organization, VA-accredited representative, attorney, claims agent, or Veterans Service Organization involved in that referral.

If you are the claimant: You are providing your own information.

If you are submitting information on behalf of a claimant: You confirm that you have the claimant’s permission or legal authority to provide this information. Proof of permission or legal authority may be requested.
